If you\u2019re having a mental health emergency or you\u2019re at risk of becoming<\/strong><\/span>\na danger to yourself or others, call 911 right away.<\/strong><\/span><\/h3><\/div><\/div>

When it comes to getting the help you need, the first step is to give us\na call to schedule a mental health assessment. Our Clinical Department is\nopen from the hours of 8:00 AM to 5:00PM, Monday to Friday. We provide\ncomprehensive intake assessments to everyone who enrolls in our\noutpatient services. We are committed to identifying and understanding\nthe emotional, psychological and psychiatric obstacles you may need to\novercome. You do not need a physician referral to receive an\nassessment from us.<\/p><\/div><\/div><\/div><\/div><\/div><\/div><\/div>

During the intake assessment, a professional staff member will meet\nwith individuals to determine which method of care will best fit their needs. When the\nassessment is completed, individuals will be connected with the services\nthat will meet their needs most effectively. We expedite direct\nadmissions from hospital and other professional entities.<\/p><\/div><\/div><\/div><\/div><\/div><\/div><\/div>

  • State-issued ID (i.e. driver\u2019s license, birth certificate,\nidentification card, etc)<\/li>\n \t
  • Health insurance cards<\/li>\n \t
  • Social Security number<\/li>\n \t
  • Bottles of any medication you are currently taking<\/li>\n<\/ul><\/div><\/div><\/div><\/div>

    • A lists of medications and dosages<\/li>\n \t
    • Over-the-counter medications and herbal supplements<\/li>\n \t
    • Hospital discharge documents<\/li>\n \t
    • Previous medical records and clinical documents<\/li>\n<\/ul><\/div><\/div><\/div><\/div><\/div><\/div><\/div>

      Any items that are considered dangerous or contraband are not allowed on\nour premises at any time. These can include sharp objects like knives\nand scissors. Any liquids that contain alcohol, drugs, tobacco, weapons,\ne-cigarettes and other items that could be considered hazardous by our\nstaff are not permitted.<\/p>\n
